Whatever your patient's care needs, you will find a complete continuum of care here at Renaissance Gardens. Before and after his or her arrival, we will:
- Send our acute-care coordinator to you. Our acute-care coordinator comes to your site; meets the patient and family; speaks with nurses, discharge planners, and social workers; and gathers all required information.
- Communicate with the patient and family. We handle all discussions about our services, invite the patient and family to tour our site, and finalize all aspects of care.
- Handle Medicare. If your patient’s care is covered by Medicare, we make all arrangements and process all paperwork.
- Physically smooth your patient's transition to our care. If you like, we can arrange transportation for your patient to our site, welcome your patient with a waiting wheelchair (as needed), escort your patient to his or her room, and do everything to ensure that your patient is comfortable and secure.
- Arrange your privileges here. If you desire to keep overseeing your patient’s care while they’re here at Renaissance Gardens – either in Short-Term Rehab or one of our longer care areas—we’ll make your access here as easy as possible.
- Maintain communications with you. Our on-staff social workers will manage on-going communications with you as needed, providing any necessary records of our care.
