We have created simple checklists to help you determine what type of care your loved one needs. There are five checklists:
Just print out the list(s) you think you need, fill it out and you will have a complete profile of your loved one’s needs. You may find you actually need a different type of care then you suspected.
Next, use the information your checklist provides in discussions and decision-making with your loved one and/or other family members, friends, and care providers.
We recommend that when you call or visit us at Renaissance Gardens you refer to these lists when speaking with our health care counselors. They can help you further in determining your loved one’s care needs.
