Q: What is the Continuum of Care?
A:
There’s a complete “continuum of care” at Renaissance Gardens. It ranges from relatively short stays (for a weekend to several months) with Respite Care and Short-Term Rehabilitation, to the more extensive services of Assisted Living*, Alzheimer’s Care, and Long-Term Nursing Care.

Continuum of care is arranged in such a way that as your loved one’s health needs change, he or she can move into different residences, with lesser or greater supervision of his or her daily needs.

It means that even if your loved one needs to go back and forth from one service to another, he or she will be cared for within a system and by professionals already familiar with your loved one’s needs.

Q: What is the first level of care?
A:
The basic types of care we provide to every resident:

  • Primary Care and Specialty Care at Renaissance Gardens
    Renaissance Gardens has Erickson HealthSM physicians who regularly visit during the day and others on call nights and weekends. The residence also features licensed nurses on duty 24 hours a day. Your loved one may also receive specialty care from credentialed Medical Center consulting specialists who visit their Renaissance Gardens patients on an individual basis.
  • Primary Care, Mental Health Care, and Specialty Care at the Medical Center
    Each Erickson community has an on-site Medical Center. If your loved one so desires, he or she may visit the Medical Center to receive primary medical care from our staff of full-time, board-certified physicians; receive mental health care from our clinical nurse specialists in mental health; and receive specialty care from consulting specialists credentialed by Erickson to provide regular office hours at the center.
  • Emergency Care
    Renaissance Gardens residents have access to the same highly trained, 24-hour-a-day emergency staff that services the rest of their community.
  • Care From Non-Medical Center Physicians
    Your loved one may have access to his or her own off-site primary care or specialty physician(s). Your loved one can leave the community to visit the doctor, or the doctor may come to Renaissance Gardens if he or she has received Erickson credentialing.
